Angel Food Order Form

Please Note:  All orders must be prepaid!
Payment must be made by cash, money order of cashier's check
(made payable to FISH Angel Food), or the Ohio EBT Food Stamp Card.
No personal checks or credit cards are accepted.

There are three ways to order:

ORDER METHOD #1 - ONLINE

Please click the link above to use your debit or credit card to place
your online order.  Then, print out your receipt and bring it with you,  
along with a large laundry basket or cooler, to pick up your order on
the distribution date.  Online orders must be place no later than
Sunday, September 14, 2008 at 5:00 p.m.

ORDER METHOD #2 - IN PERSON
Complete the order form (click the link above on this page)
and bring it along with your cash, check or money order
(made payable to FISH Angel Food) to:
Wadsworth Public Library
Wednesday, September 10, 2008, from  10:00 a.m. to noon
and
Thursday, September 11, 2008 from 6:00 to 8:00 P.M.

ORDER METHOD #3 - MAIL ORDER
Mail Orders are also accepted and must include a completed order form
(click on "Angel Food Order Form" button on this page), a money order
or cashier's check (made payable to FISH Angel Food), and a
self-addressed, stamped envelope for return of your order receipt.
No cash or personal checks will be accepted on mail orders.  
May mail orders must be received NO LATER than Friday, September 12, 2008.   
Mail orders received after 5:00 p.m. on September 12, 2008 may be returned.

Angel Food Ministries reserves the right to substitute any of the above items
due to availability, cost and quality.

    Since November, 2006, Wadsworth FISH has been a proud host site for Angel Food
    Ministries,  a non-profit, non-denominational organization dedicated to providing delicious
    groceries and financial support to communities throughout the United States. The program
    began in 1994 with 34 families in Monroe, Georgia, and has grown to serve hundreds of
    thousands of families every month across 35 states.  






    Angel Food groceries are the same high quality one could purchase at a grocery store.
    There are no second-hand items, no damaged or out-dated goods, no dented cans without
    labels, no day-old breads, and no produce that is almost too ripe.  Angel Food offers your
    family delicious frozen meats - steak, beef, chicken, pork, ribs - as well as frozen and fresh
    vegetables and a variety of boxed items, with a menu that changes monthly.







    And, Seniors and individuals - check out the new Senior Box available beginning in
    September!  Geared specifically to seniors and individuals,






    Once either or both of these special boxes are purchased, you can also order as many of the
    monthly specials offered that month at a nominal additional fee.  There is enough food in the
    Basic Unit to feed a family of four for about 1-2 weeks or an individual for up to one month.








    There are no applications to complete or qualifications to which participants must adhere.  
    This program is for EVERYONE and provides delicious food at an amazing savings.   We
    accept cash, money orders, and the Ohio EBT card.   Online ordering is also available for
    use with your debit or credit card.

Place your order the second week of the month and pick it up at our distribution
site typically the last Saturday of the month ------
when there is more month at the end of your money!
There is no limit as to the number of months you may participate.  Be assured of
having delicious food sufficient to feed your family well into the next month!
